1.jpg 2.jpg A 55-year-old woman visits your practice complaining about a painful tooth (36). She is an old patient of yours, but hasn’t visited you in several years. On intra-oral examination you detect an unsound amalgam restoration on the 36. She also presented with white patch on the left lateral surface of the tongue. History Main Complaint The patient complains about a fractured filling on the 36 that is starting to become painful. She recalls the tooth breaking while she was eating a “KFC” chicken bone about 2 weeks ago. History of Complaint You only noticed the lesion on the tongue for the first time during your clinical examination. There is no written record of the white patch in her dental records. The patient reports that she noticed the lesion but had ignored it, as it was not painful. She thinks it has probably been there for the last year. Medical History The patient is otherwise fit and well. She smokes 4 cigarettes a day and drinks 4–8 glasses of red wine a week. She was that she was told by a friend that a glass of red wine helps prevent heart disease. Examination Extraoral Examination No lymph nodes are palpable in the neck, and there are no abnormal findings on extraoral examination. Intraoral Examination Apart from the unsound restoration on the 36 and the lesion on the tongue, the remainder of the oral mucosa is normal. Based on the information above, formulate a differential diagnoses from the list below (1 being most likely to 4 being least likely).
