Pаrt 1 оf 2 pаrts Incоrrect, incоmplete аnd illegible physician orders can lead to treatment delays and jeopardize patient safety.  Mistakes on orders include incorrect patient’s name, incorrect patient’s contact information, incorrect insurance plan information, missing diagnosis, missing referrals, and other mistakes.  As part of a project to reduce treatment delays, staff identified 10 pieces of critical information on a physician’s order.  A mistake in any one of these 10 pieces of information is a defect, and any single order may have more than one mistake.  The project team reviewed a sample of the prior month’s orders to establish baseline performance.  In a sample of 250 orders, a total of 255 mistakes were found on 150 orders.
