A 45-year-old woman was diagnosed with a primary brain tumor 3 months ago and has started outpatient chemotherapy. Her last chemotherapy visit was a week ago. This week, her husband noticed that she was less alert, and not interested in doing any activities. Today, she was found unconscious in her chair. Her husband called an ambulance to take her to the Emergency Department. The nurse records these assessment findings: Vital Signs: Temperature 98.9°F (37.2°C), blood pressure 190/90 mm Hg, pulse 54 beats/minute, respirations 10 breaths per minute and irregular. Pulse is full and bounding. Unconscious and does not respond to voice; her husband told the nurse that she was talking last night. Skin warm and dry. Trace edema notes on the ankles bilaterally.
