8. 1200: Pаtient аdmitted tо unit fоllоwing MVA with evidence of closed frаcture to the right humerus.  Alert and Oriented X 3.  C/o pain of 4 on scale of 1-10.  Neurovascular assessment with small amount of swelling, no further abnormalities noted.  Lung sounds clear anteriorly, posteriorly, and laterally.  Heart rhythm is regular at 68 BPM.  Skin is warm and dry to touch.  Turgor recoil less than three seconds.  Bowel Sounds audible X 4 quadrants.  Bladder nonpalpable.  States voiding without difficulty.  Last BM 2 days ago was large, soft, and brown in color.  Splint intact and right arm elevated on pillow.  S. Nurse, SPN 0200: Patient c/o  pain of 6 on scale of 1-10.  Sharp, stabbing and radiating to phalanges.  Administered Demerol 50mg in Rt Ventrogluteal using sterile technique.  Tolerated procedure without complaint.  S. Nurse, SPN 0240: Pain level 6/10.  Comfort measures performed.  Stated, "I am a little sick to my stomach." Placed cool cloth to neck.  Stated, " That feels better. I think I will be okay."  Instructed to notify nurse if any further need arises.  Verbalized understanding.  S. Nurse, LPN 0420: Patient c/o pain 10/10.  Noted capillary refill appx 5 sec.  Extremity pale and cool to touch.  Unable to palpate pulse to affected extremity.  Paresthesia noted.  Stated, "I am not feeling well at all.  I do not think I can take this any longer."   Which of the following interventions will the nurse perform first?
