STOP DO NOT PROCEED WITHOUT READING THE INFORMATION BELOW AND AGREEING TO THE STATEMENT. You have 80 minutes to complete the 65 questions in this exam. This exam is CLOSED EVERYTHING which means it is CLOSED NOTE, CLOSED PRINTED MATERIALS, CLOSED TEXTBOOK, CLOSED DIGITAL RESOURCES AND YOU MAY NOT TALK TO ANYONE DURING THE EXAM OR AFTER ABOUT THE EXAM. Utilizing any resources or talking to other students is not permitted and will be considered academic misconduct. All materials related to the course, electronics (i.e. smart watches, computers, smart phones, tablets, headphones etc.), books, printed materials, backpacks, etc. must completely out of sight during the exam. Please turn off all cellphones and watches before beginning the exam. Note taking is not permitted. Location: You are permitted to take the exam at the location of your choice. However, you may not have anyone in the room with you while taking the exam. You are required to find a room that is private with no distractions. HonorLock: HonorLock is enabled while taking the exam. You are required to have your webcam and microphone working throughout the entire exam. Your full face needs to be visible at all times. Not showing your entire face on camera during the exam will result in identification of suspicious activity and you will be reported to COAM. No Recording or Photographing: You may not under any circumstances record or photograph the exam. Access to Calculators: You may use the basic calculator provided in HonorLock during the exam. No other calculators are permissible. Questions During the Exam: Students are not permitted to ask the instructor or proctors questions during the exam. To be fair to everyone, absolutely NO SUBSTANTIVE QUESTIONS about the exam material will be answered. Leaving the Room During the Exam: Students are not permitted to leave the exam unless there is an emergency situation. Please be sure to use the restroom prior to the start of the exam. If you have an emergency situation and need to leave the exam to use the restroom, then we kindly ask that email (FCOB-busml3150@osu.edu) immediately following the exam to notify us. If you fail to notify us then your exam score will reflect the questions you answered at the time you left the view of the computer webcam. If you have a medical condition that would require you to leave the exam for any reason, then you are asked to register with SLDS before the exam and you should be taking your exam at SLDS’s office on campus. Second-language students who need a translation dictionary must email (FCOB-busml3150@osu.edu) at the beginning of the semester by the second class period to make arrangements for use of a dictionary during the exam. If you have not communicated with the instructor regarding your need for a dictionary, then you are not permitted to use a dictionary during the exam. You may not use a personal dictionary (printed or electronic). Food and Water: You may have a water bottle with you during the exam. Eating is strictly prohibited during the exam. If you have a medical condition where eating is necessary, you should have made arrangements with SLDS for such accommodations and the instructor must be notified via email FCOB-busml3150@osu.edu prior to the start of the exam. By selecting one of the answers below you consent to the policies listed above and the following: Academic Misconduct Clause: I understand that if my instructor suspects that a student has committed academic misconduct during the exam, they are obligated by University Rules to report their suspicions to the Committee on Academic Misconduct. If COAM determines that the student has violated the University’s Code of Student Conduct (i.e., committed academic misconduct), the sanctions for the misconduct could include a failing grade in this course and suspension or dismissal from the University. Nondisclosure Clause: I hereby swear that I will not speak of this exam to anyone in any terms this semester or in the future. This includes even mundane topics, such as the number of questions on the test, when the test was held, or the lighting in the room. The only appropriate response to a question about the exam is, “I cannot comment.” I swear that I have not spoken to anyone about the contents of this exam. Property of the Exam Clause: I agree that this exam is the property of the professor, Claire Adams, and that I will not copy any of its contents. I have a right to review this exam after it has been graded. I agree that if I copy this exam or if I walk out with this exam, I will get a zero on the exam and will be submitted to COAM for academic misconduct. Computer, Calculator, and Cell Phone Clause: I agree to turn off all digital devices aside from the computer I am using to take the exam. This includes but is not limited to phones, tablets, watches etc. If I am taking the exam and my cell phone rings, vibrates, or makes a noise, then I will wait to answer it until the exam is over. The only materials in addition to my computer on my desk at the time of the exam is a water bottle if desired. I will not eat during my exam.
