Mоlly presents tо the clinic fоr а sleep аpneа follow up. She started CPAP about 2 months ago and has not been able to use the device. Her biggest concern is chronic nasal congestion as well as feeling smothered and suffocated initially with the device on. Her pressure is set in the auto mode at 6-16 cmH2O, ramp set at 25 minutes at a pressure of 4 cmH2O, humidity set in the auto mode. She is using a nasal pillow mask. At times, she is opening her mouth to "get air" and is getting dry as well. Her compliance shows average nightly use of 20 minutes 30/60 nights. Her leak is 45 L/min. Her pressure 95th percentile is 5.4 cmH2O. What do you think is the primary issue for Molly and what would you suggest to help her reach compliance in her last 30 days of her trial period?
