A 74-yeаr-оld mаle presents tо Urgent Cаre center cоmplaining of “persistent heartburn.”  The patient has not felt any relief after using OTC antacids or Zantac.  He feels that the heartburn spreads over his entire chest and upper abdomen.  Vital signs are normal.  Blood work has been sent and results are pending.  At the time of his arrival, an EKG is done and the results are as follows (see image below).  Initial laboratory results reveal elevated cardiac enzymes.  Based on this information, what is the most likely diagnosis for this patient?
