Susаn is а 32-yeаr-оld White wоman currently wоrking as a computer technician. She has two siblings: an older brother and a younger sister. Her parents are divorced. She has minimal contact with her family. At present, she is living with her boyfriend, but states that he has asked her to leave. She is not sure if she can afford to live on her own and does not have any friends that she feels she can turn to. At work recently, she has had more difficulty completing her duties, as her ability to concentrate on tasks has been diminishing. Her supervisor has noted her poor performance and is recommending that she be laid-off during next week’s “corporate downsizing.” Because of the recent difficulties, she has been struggling to get out of bed in the morning and feeling “overall sluggish.” In your discussion with her she describes her conditions as “more so than usual.” Susan explains that her personality, in general, is “modest, even subdued.” When pressed for additional details, she explains that since early adulthood, she has never really been happy, nor has she maintained regular sleeping patterns or had much interest in social activities (“or much else, really”). She says she regrets not having made more friends or “living life to the fullest,” and she views her current situation as her “lot in life, just how things are.” She believes friends would be of great comfort to her during this difficult time. She has always wanted more friends but feels she does not communicate well with people and as a result she tends to avoid social situations. She states, “I may not have friends to help me, but I’ll happily take the unemployment payments, food stamps, or any other assistance that’s available to me – so, maybe I’ll be okay.” In discussing her overall health, Susan explains that she has a history of colitis, which was in remission until about a week ago when she began having symptomatic flare-ups of the disorder. She describes the symptoms as disabling. _____________________________________________ Please respond to the following prompts: 1. For many different professions, ethical behaviors are guided by principles such as Autonomy, Beneficence, and such. The counseling profession, however, is defined by several unique core values. What are three core values of the counseling profession and how will they affect your work with Susan? 2. Susan states that she is willing to accept government assistance (“unemployment payments, food stamps…”). If you were to have a strong moral opposition to accepting governmental assistance (based upon the belief that everyone should take care of themselves), how would you manage this conflict in personal values (i.e., she is accepting government assistance whereas you [hypothetically] believe that is not the best way to live one’s life). 3. Considering the symptoms presented, what provisional diagnoses would you assign? Please begin your answer by clearly listing the diagnoses that you think are appropriate for her. Her diagnosis should be complete (more than just the primary clinical concern – it should include all clinically significant issues, including psychosocial factors). After listing your complete diagnosis, briefly explain your rationale, noting symptoms and how she meets the criteria for such diagnoses. Note: You do not need to list the actual numerical codes. 4. Counseling Theories Section: Please answer Option A or B (not both). Option A is comprised of 3 subsections (A.1., A.2., A.3.). If you are choosing Option A, all 3 prompts in the A section need to be addressed (i.e., you must answer A.1., A.2., and A.3.). If you are choosing Option B, there is only one prompt (B.1.). A.1. You decide to take a Person-Centered approach. Central to Person-Centered counseling is Carl Rogers’ “Necessary and Sufficient Conditions.” Please list and explain these Necessary and Sufficient Conditions and how they will be useful in addressing Susan’s conditions of worth. A.2. You decide to use a Cognitive Behavior Thought Record. Please create a possible cognition that Susan’s may have -- a cognition related to her current concerns -- and explain the Thought Record process, step-by-step, including how it could be applied to her current state. To do this, please be certain to use CBT language in your response, naming the specific type of problematic cognition or cognitive distortion that Susan may be employing (you can create a hypothetical thought that Susan might have). A.3. To help her with her social struggles, please explain a specific counseling technique from any other theoretical orientation (other than Person-Centered and CBT) that could help her. To do so, please identify the theory from which this technique comes and provide details of how the technique would be used with Susan’s social struggles. -OR- B.1. You decide to work with Susan by adhering to one counseling theory. Please provide details regarding your chosen theory, including how does the theory explain the development of problems (and strengths), how are they are maintained, and how are problems alleviated. Also please apply the theoretical concepts to Susan’s experiences and utilize at least two interventions that are central to the theory. 5. In working with Susan, please apply a developmental theory to your case conceptualization and clinical work. Please type your answers in the field below.
