BASED UPON MARVIN’S CASE Occupational Therapy initial evaluation results: Cognition: A & O x 3, intact memory, no perceptual deficits Sensation: intact ROM: WNL for all extremities, neck and trunk passively. Bilateral plantarflexion contracture of -5 degrees of dorsiflexion on the L and -10 degrees on the R Balance: sitting balance is normal, standing balance is moderately impaired. Requires a FWW and min A to attain and maintain upright. Does not tolerate more than minimal challenges to his balance in standing with a FWW Pain: moderate, frequent complaints of LE muscle stiffness and cramping Transfers: sit to stand with min A and FWW; bed to wheelchair with min A with FWW; wheelchair to commode min A with FWW Functional mobility: independent in wheelchair set-up, propulsion and maneuvering on level surfaces; minimum to moderate A to ambulate with FWW & fatigues easily Self-care: Independent from seated position with feeding, grooming, and UE dressing. Able to complete most LE dressing, requires mod A for standing to don pants over hips. Mod A for toileting.
