Ed Murphy is a 66-year-old who presents today with complaints of chest pain. He states that it is left sided and gradually worsening over the last 48 hours. He has obtained no relief with rest, food, exercise, Mylanta, or Tylenol. He states it hurts all the time and is worse when his chest touches something and that when he took a shower this morning, the hot water hurt that same area. He states it is waking him up at night. He is on no routine medications, normal vital signs and exam. What is your most likely diagnosis?
