KOHLMAN EVALUATION OF LIVING SKILLS Mrs. T is аn 82 yeаr оld femаle whо was recently hоspitalized with a mild CVA (stroke). Prior to this event, she lived alone in an assisted living apartment. She received assistance with weekly housekeeping and one meal a day. For an additional fee, she is able to receive up to two meals a day and assistance with showering. In order to return to her apartment, she needs to be independent with: basic activities of daily living, light meal preparation, walking to get around her apartment, and getting to/from the dining room. She will be discharged home with a quad cane. Her son and his family live in the same town and provide intermittent assistance, especially with the management of her finances and bills. Mrs. T would like to return to her apartment. The Kohlman Evaluation of Living Skills (KELS) was used to evaluate Mrs. T’s ability to return to her assisted living apartment as she is nearing the end of her inpatient subacute rehabilitation stay.
