For the client below, which items are risk factors for, signs or symptoms of Alcohol Use Disorder: History of Present Illness: Client presents today for follow-up on their recent diagnosis of gastritis and gastrointestinal bleeding. Review of systems: Reports diplopia. No ear pain or discharge but has some nasal congestion with intermittent clear discharge. Pain, burning, and tingling of extremities noted. Client complains of headache. Complains of peripheral edema and mild shortness of breath with walking. Client reports some burning with urination. Medical/surgical history: Hypertension for 3 years, obesity, acne, right wrist fracture at age 13. Social history: Denies smoking cigarettes. Reports drinking around 3–4 beers when “out partying” with coworkers at a local bar. Denies illicit drug use. Family history: Father and mother with history of alcoholism. Mother with coronary artery disease and anxiety disorder. Father with peptic ulcer disease. Brother has a history of smoking cigarettes. Physical assessment: Overweight male with a BMI of 40. Alert and oriented. Mild diaphoresis. Thyroid enlargement noted. Lungs clear; S1, S2 heart sounds without murmurs or extra beats. Noted 1+ edema of both lower extremities. Ataxia noted. Medications: Atenolol (beta blocker) 10 mg daily.
