Tоm is а 19-yeаr-оld fоotbаll player with a six-month history of intermittent shortness of breath with exertion. While working out for preseason conditioning, he complains of fatigue, tightness in his chest, and difficulty catching his breath. He has recently had an upper respiratory infection, fever, cough, and congestion, all of which are resolving. He also complains of mild allergy symptoms.   Tom reports that he is not on any medications. His medical and family histories are negative for any respiratory issues. His vital signs are as follows: temperature, 98.6 °F (37 °C); respiration, 14; pulse, 62; blood pressure, 100/62. On examining the head, ears, eyes, nose, and throat, the only finding is congestion in the sinuses.
