In the questions below, enter the Medication-Related Problems (MRPs) and recommendations to resolve them. You may include up to 6 MRPs. List only one MRP per question. If you have fewer than 6 MRPs, the remaining questions should be left blank. For each MRP, you must include two things in your entry: Clearly describe the problem and evidence that supports why it is a problem Provide a clear and specific recommended solution to the problem; for drug changes/additions, this must include a SPECIFIC drug/dose/frequency and duration if applicable Tips: EXTREMELY IMPORTANT: As noted above, you MUST be specific (drug/dose/frequency/duration if applicable) when making ANY recommendation for a drug change. Failure to do so could result in an unsatisfactory performance on this station. You do NOT need to include the priority (high/medium/low), and you do NOT need to list them in order of priority.
