Jason is a 45 year old male who works as a high school english teacher. His lives at home with his wife, no kids. He enjoys reading. He has self-diagnosed himself with anxiety and recently reached out to his physician for a referral to see a psychologist. At his physician’s visit, they drew labs and did a physical. Jason talked to his physician about the following: He father died 6 months ago from cardiovascular disease Jason is stressed with his fathers death and in addition to that he is handling all the of this father’s matters (life insurance, selling of his house, etc) following his death. Jason also has increase worry about his health. He describes his diet as a lot of pre packaged meals or quick freezer meals that do not require a lot of cooking. Yesterday he ate: 2 slices white toast with butter, 3 sausage links and 2% milk to drink for breakfast, hamburger on white bun, fries, and apple for lunch with water, frozen chicken pot pie and pop for dinner, corn chips and salsa for snack. He has been thinking more about his diet following his father’s death and is thinking about making a change. He has started walking about a month ago. His physician sends him to a health coach and asks him to track his sleep and activity. He is here to see you with the following information.
