ScenаriоAccоrding tо “Heаlth & Democrаcy,” access to health care in South Africa is a constitutional right. Civil society plays a crucial role in promoting accountability in medicine policy. Under the proposed NHI system, South Africa aims to reduce inequities in health care access. The plan includes a uniform benefits package, strengthened EML adherence, and centralised procurement. This plan would be achieved through the implementation of a Health Technology Assessment (HTA) agency that must regularly review a range of health interventions and technology by using the best available evidence on several factors.While attending the provincial Pharmaceutical and Therapeutics Committee (PTC), the committee is reviewing antimicrobial (including antivirals) usage trends, concerned about irrational prescribing, and the pressure to approve the use of new antimicrobials that are not on the national EML. The committee decided to write a proposal to the National Essential Medicines List Committee (NEMLC) to include a new antiviral on the EML. The antiviral has been recommended by international guidelines but has a higher acquisition cost than available alternatives. The HTA process is being used to determine if the medicine is clinically effective and cost-effective for public sector use. In the proposal, the PTC considers using compulsory licensing to import or manufacture a generic version of the antiviral as they fear that even other generic medicines can be unaffordable for many households.Before the meeting concludes, Mr Hlatswayo shares with the committee a report from IMPACT, which revealed an increased circulation of counterfeit antimalarial drugs in sub-Saharan Africa, highlighting the importance of having an improved post-marketing surveillance system.
