T.K. is a 4-month-old boy. T.K. did not pass his newborn hearing screening with OAE or ABR. His family reports that a grandparent, uncle, and cousin have had significant hearing loss since birth, with no other co-morbidities. T.K. was seen for a diagnostic ABR. His audiogram (based on the ABR) and tympanogram are below. OAEs were absent in the right and left ear. What is your best diagnosis for T.K.?
