The patient is a 55 yo female admitted to a skilled nursing facility after a prolonged hospitalization for chest pain. The patient was diagnosed with 4 vessel coronary artery disease and aortic valve insufficiency and underwent a coronary artery bypass graft x 4 and an aortic valve replacement. Upon discharge from the hospital, the pt required mod assist for bed mobility, max assist for sit to stand transfers due to sternal precautions, and can ambulate 20’ with RW and minimal assistance. Pt becomes short of breath (SOB) with minimal exertion which is why gait distance is limited to 20’. Pt lives with her dog, Maddie, in a 3-story home with stairs. She works in a medical lab and was independent in all ADLS and IADLS prior to surgery. She does not have family in town but has friends who are willing to assist intermittently once discharged to home. Sternal precautions: cannot reach both arms back behind to stretch sternum, not allowed to use UEs to push to stand, 5 lb lifting restriction, no overhead work. They last x 8 weeks.
