A 34-yeаr-оld femаle presents tо the оffice with severe pаin on defecation for the past two days. The patient states that she had not had a problem with bowel movements in the past. Recently she had noticed some increasing constipation when she started taking some new daily vitamins. Now she has had three bowel movements in the past two days, and she states that “it felt like she was being cut with glass” each time she passed them. The pain was excruciating. It usually diminished in intensity when the bowel movement ended, but the anal area throbbed for 1-2 hours afterward. There was a small amount of blood noted when wiping, but none noted in the toilet. The patient's past medical history was negative. She took no prescription medications. There was no family history of colon cancer. On examination, you would expect to find:
