Reаd the fоllоwing ED dоcumentаtion. Whаt is the patient's Chief Concern? Emergency Departiment Notes: History of Present Illness: The patient is a 16-year-old male with Type 1 Diabetes and ADHD. Earllier today, he was playing baseball in the park. Three hours prior to presentation, he stated that he started to slide into home plate, and someone stepped on his right ankle and kind of twisted it. At this time, he denies being able to bear weight on it. He states that it hurts to move or stand on it. He does not remember any injuries to this ankle in the past. Medical Decision-Making: An X-ray of the right ankle showed a small ossicle versus avulsion fracture of the talonavicular joint on the lateral view. He has had no pain over the metatarsals themselves. This may be a fracture based upon his exam. He was given Motrin here. He will be discharged home to follow up with Dr. Torres from Orthopedics next week. Assessment: Acute ankle sprain. Disposition: Splint was administered, and patient was supplied with crutches at discharge. He was given a prescription for Motrin and some Buprenorphine if necessary to reduce pain to sleep. He is given instructions to follow up with Dr. Torres. Return if any worsening problems.
