You saw him several months ago for the same thing and advised, rest, ice/heat and tylenol. Patient reports no change in symptoms. What is the next best step in for this patient?

A 55 yeаr оld mаle whо wоrks аs a plumber complains of fatigue, morning stiffness and pain in both hands and wrists upon wakening which improves after about 2 hours. He reports some areas of warmth and inflammation but that seems to improve throughout the day. He denies any recent trauma or injury to the area. On exam you note bony prominence on the metacarpophalanygeal joints bilaterally with redness and tenderness to palpation. There is also palmar erythema.
