Yоu hаve just received аuthоrizаtiоn to provide behavioral services for Erik, a 15 year-old male. The only information that you have received regarding your new client is a short background narrative provided by the support coordinator who referred the case to you: "Erik is a 15 year-old white male who currently resides at his family home in Red Rock, WY with his adoptive mother and step-father. Erik has lived at home with his adopted mother since February 2, 2010, when he was legally adopted. Erik previously lived with his biological parents until the age of 22 months and was then transferred between foster care placements until he was adopted. Erik attends a private school, Super Cool Private School, 5 days a week. At school, Erik participates in a blended-learning curriculum that consists of both in-person as well as online instruction. Erik is currently in the 8th grade and requires 1:1 staffing 5 days per week throughout the school day. Erik’s current respite worker has been providing 1:1 services at school for approximately 3 weeks. Prior to that a different respite worker provided these services. Erik’s school has reported that Erik is very behind in his academic work (e.g., performing near the 2nd grade level in some areas). Additionally, his school has reported frequent behavioral concerns. Specifically, Erik has a history of engaging in serious problem behavior at school including physical aggression, property destruction, elopement, and clogging toilets. These problem behaviors have resulted in monetary damages in excess of $5000, police involvement, mandatory psychiatric holds, as well as, suspension and expulsion. Additionally, due to the severity of Erik’s problem behavior, he was removed from the classroom and spent most of his school day receiving 1:1 services and academic instruction from his respite worker in the school’s library. Erik has been gradually reintroduced back into the classroom and now only spends a small portion of his day receiving 1:1 services and instruction in the library. Erik also reportedly engages in high rates of noncompliance/malingering and disruptive behavior (e.g., walking around, touching and taking things that are not his/pocketing items) while at school, which has interfered with his academic and social learning. Erik has also reportedly previously engaged in threats of self-harm and threats of harm to others. Erik receives psychiatric services through the Red Rock Mental Health Center, which includes administration and monitoring of psychotropic medication. Erik’s primary support system at home includes his adoptive mother and step-father. Erik also receives support from his adoptive sister, who takes care of him Monday through Friday after school from approximately 3 PM to 6 PM. Erik previously had scheduled visits with his adoptive father, however, these visits were discontinued after Erik expressed that he no longer wanted to visit him. Erik’s adoptive father continues to provide some financial support and has been occasionally called to school when Erik engages in serious problem behavior." Given this information, your supervisor has asked you to write, in no more than 1,000 words, a detailed plan as to how you will complete the intake process for your new client. Be sure to provide this information in a narrative form similar to what is described above. Additionally, you should include specific descriptions regarding from where and from who this information will be obtained.
