All survey responses (including personal information) will be kept strictly confidential. No personally identifiable information will be reported back to the requestor. Your responses are combined with those of many others and summarized in a report to protect your anonymity. The data will be reported only in the aggregate and no individual will be identified. In rare circumstances, the Director of Continuing Education may access individual student information in order to address any specific concerns. The following questions are about your co-instructor for this course. Please answer these questions only as they relate to your assigned co-instructor, even if you interacted with other co-instructors during the course (e.g., you attended another meeting as a make-up, a scheduled meeting had a substitute, etc).
