Mr. Graham is a 56-year-old male who presents to your office with a complaint of sudden onset of epigastric pain and abdomen tenderness starting 3 days ago. The pain started as mostly dull and constant and now radiating to his flanks, to his back and shoulder. In the last 12 hours, he reports worsening nausea and vomiting associated with waves of excruciating knifelike pain. He denies exposure to sick contacts and recently traveled to Las Vegas for a long weekend bachelor party. These symptoms are consistent with a patient that most likely has:
