Tomas is an 82-year-old male who fell at home, sustained a hip fracture, and had surgery to repair his fracture hip. After surgery, Tomas was discharged from the hospital to a rehab facility. He had two weeks of rehabilitation with an OT and PT and is now ready to be discharged home. He will continue to need PT once home to help him return to his prior level of function. He has limited support and is currently unable to drive. What level of care would be appropriate once discharged from rehab?
